Bristol Intensive Support Service Referral Form
1625ip offer a high support placement in a self-contained flat for young people leaving care in the Bristol area.
This service is for care leavers aged 18 or over with multiple or complex needs.
We offer an accommodation placement in a 1-bedroom flat under licence, receiving 10 hours of support per week with a named keyworker. The keyworker will meet with the resident on a regular basis to provide support with building day-to-day skills for independent living.
Support activities are tailored to suit the individual, but can include food shopping, budgeting, paying bills, managing health care appointments, exploring hobbies and personal interests, and building a network in their community.
The expectation is that the young person will leave the service having secured a property through Homechoice, and with the skills to manage this successfully.
Please note: Due to the small size of our service, we are unlikely to be able to accommodate requests for a specific gender of worker.

Independent people will use the information that you have provided in the referral form to help us make a decision about whether we are able to offer support to a young person.
We will not disclose the information in this referral to any other person or organisation, unless under a legal duty to do so, or if withholding information would pose a significant risk to the person being referred.
