Bristol Intensive Support Service Referral Form


1625ip offer a high support placement in a self-contained flat for young people leaving care in the Bristol area.

Young people can be referred to the service between 16-17 of age.

As well as an accommodation placement in a 1-bedroom flat under licence, receiving 10 hours of support with a named keyworker. The keyworker will meet with the resident on a weekly basis to provide support with building day-to-day skills for independent living. These include food shopping, budgeting, paying bills, managing health care appointments, exploring hobbies and personal interests, and building a network in their community.

The expectation is that the young person will leave the service having secured a property through Homechoice.

Due to the small size of our service, we are highly unlikely to be able to accommodate requests for a specific gender of worker.

We will not disclose the information in this referral to any other person or organisation, unless under a legal duty to do so, or if withholding information would pose a significant risk to the person being referred.
