Street Dreams media consent form
This form is about giving permission for your photo(s), audio recording(s), or stories to be used in our project Street Dreams. It’s important that you understand what you’re agreeing to, so please complete this with a trusted adult (like a support worker or key worker) who can help explain anything.
About the Project
Street Dreams is a creative project working with young people to explore and share their experiences through photography and audio storytelling.
Young people will be working with Colin Moody to create the photos.
Young people involved will also have the opportunity to take some photographs themselves. Some photos will be of young people, and some may be both taken by and of young people.
There is also an audio element, where young people can share their stories in their own words, which will be featured alongside the photography.
Photos and/or audio stories from Street Dreams may be:
• Displayed in public exhibitions, galleries, or in the new Kingsley Hall building
• Shared online (e.g. social media, website, e-newsletters)
• Used in printed materials (e.g. posters, leaflets)
• Shared with the press or organisations we work with
• Sold (photos only – with any money raised going towards supporting young people)
All photos created for the project will be owned by 1625 Independent People, and we may use them again in the future to promote the work of the charity.
Please be aware: if your photos or stories are shown publicly – for example, at Kingsley Hall, in galleries, or online – they might be seen and shared by others (like on social media). Once this happens, it’s outside our control and may not be possible to remove later.
